Severance's Innie and Outies, Mark and Helly have many complications that have forced their relationship to take a different route.
For instance, Mark discovered that his Outie had a wife named Gemma whom he thought was dead. This wife was revealed to be Lumon's Ms. Casey, who is linked to Innie Severance's Eagan family tree and the heir to Lumon itself, Helena Eagan. All of this made Mark and Helly's relationship - a love triangle, when counting Ms. Casey - more complex than it was, but Severance season 2, episode 4 just added another twist that somehow makes it even worse.

A Big Season 2 Theory Was Confirmed & Led To Unexpected Outcomes
A common theory of Severance season 2 thus far is that Helly never returned to Lumon's severed floor and that it has been Helena Eagan all along. Severance season 2, episode 4 confirmed as such, though not before she and Mark slept together during one of Lumon's work incentives, this time being a camping trip. Mark, thinking Helena was Helly, entered her tent and the two had sex while Dylan slept in another tent and Irving slept in the woods, having argued with Helena earlier.

Many little details from Severance season 2 episode 1 have left viewers wondering whether Helly is actually who she claims to be or if she is Helena.
By the end of the episode, Irving's suspicions that Helly is Helena are confirmed and Milchick removes something called the Glasgow Block. This brought back Helly right at the end of Severance season 2, episode 4, confirming to Mark that the woman he slept with was Helena. Rather than the already complex love triangle - or even square - of Innie Mark, Outie Mark, Helly, and Gemma/Ms. Casey, Helena is now added to the mix.
Helena watching the Severance footage of Helly and Mark's kiss proved that she longed for that kind of connection, with episode 4 showing her willingness to act on it with Mark. Helena may wish to continue pursuing such a relationship, be it with Innie or Outie Mark, going forward. Then there is the added complication of Mark thinking he was sleeping with Helly when in reality it was Helena. Overall, the moment adds a heap of issues and complexity that were not prevalent before in Severance seasons 1 and 2.

Relationship Problems Are On The Horizon
Naturally, the burning question the audience has after Severance season 2, episode 4 is what this development means for Helly and Mark's relationship. There will almost undoubtedly be some difficult conversations, especially if - or, more likely - when, Helly finds out that Mark and Helena slept together. Mark unknowingly did so, but Helly likely resents Helena and what she stands for, meaning she could feel betrayed that he did not know the difference between the two of them.
Moreover, Mark himself will likely feel violated by the act. Helena used the guise of Helly to sleep with Mark, which could lead to some trust issues between him and Helly further down the line. If Helena has been acting as Helly since the very beginning of Severance season 2 also, the latter does not know about Ms. Casey/Gemma, which is sure to cause even more discussions. On the whole, Severance season 2, episode 4 has likely changed Mark and Helly's relationship forever, going way beyond the complications of a love triangle.
