In the previous episode, Mob had been trying to arrange a date with his crush, Tsubomi, before she leaves town for good. On his way to meet her, Mob saved a child from being struck by a car by diving in front of it, taking the hit instead. Mob apparently didn't even consider using his psychic powers, which he had previously used to save a cat from the same fate. Bleeding out on the ground, the episode ended. Episode 10 of Mob Psycho 100 begins the same way that 9 had ended, but Mob gets up, and that's when things really take a turn for the worse.
As the people around try to get Mob medical help, the ground cracks all around him, as if a massive force was released, leaving behind a crater. Mob rises, and it's immediately clear that his ???% mode, which only comes out when he's in a life-or-death situation, has emerged. This has the unfortunate consequence of typically rendering Mob's true personality unconscious, letting the alter ego take control of his body entirely. Mob's subconscious begins leading him through the city with his powers running wild, destroying roads and buildings as he simply es by. Teruki Hanazawa, Mob's friend and a fellow powerful esper, tries to stop his rampage but is absolutely obliterated, ultimately sent flying for thousands of feet and crashing into a building. Others also try to put a stop to this, but are unlikely to even slow him down.
Mob Psycho 100 Finally Explains Why Shigeo Fears His Powers
Knowing that Mob has been capable of such destruction all along and simply held it back and suppressed it radically changes the series. Mob has come around on accepting his powers to some extent, but ever since the beginning he's been afraid to truly embrace them. In the same way that someone who is suppressing their anger might eventually explode with rage, Mob's original personality is still conscious, but is completely unable to take control of the body back, forcing him to watch helplessly as his friends are beaten and the city is destroyed... and it's all his fault, as far as anyone is concerned.
In order to make sure this can't happen again, Mob Psycho 100 will have to see Mob coming to accept himself entirely, powers and all. That is, of course, assuming that Mob is able to regain control--and that's far from a given this close to the end.
