There are 20 different soulstones spread across the first major region in The First Berserker: Khazan (Charnak Mountain Range), and these are just the first of many you'll be able to break over the course of Khazan's journey. Charnak Mountain Range consists of six major areas, each with between two and three soulstones scattered throughout their various levels. You might have to do a bit of looking around to track down some of these, while others are extremely obvious and cannot be missed.
Soulstones are exchanged at Daphrona in the Crevice for enhanced Lacrima gain, which is Khazan if you need Lacrima.

Break All 20 Soulstones In The Charnak Mountain Range
There are 20 different soulstones in the Charnak Mountain Range, and you can track them all down pretty easily, assuming you know where to look. I've put together a little cheat sheet here to help, but a more detailed breakdown of each location is found below. Many of these can be found naturally while exploring the various areas, but there are often barricades or other obstacles that you might not give a second glance, which are actually blocking hidden paths that lead to these soulstones. Be sure to explore each area thoroughly, and be grateful none of these are hidden behind invisible walls.

Where To Find Every Soulstone In Charnak Mountain Range
Mount Heinmach Soulstones
There are two Soulstones in the Mount Heinmach starting area. The first can be found at the Frosty Cave's Entrance Blade Nexus after defeating the first mini-boss of the game. You may not have javelins yet, so you can take the path on the left until you run into a couple Pell Los Swordsmen. Take them out and drop down to the right to follow the path up. The second Soulstone is found by following the path to the right after defeating the Granite Jocko enemy.
Storm
The first Soulstone in Storm is in the Snowfield of Trials just after where you first learn to use the Javelin. The second is after the Frozen Lake Nexus, just past where you pick up the "An Imperial Soldier's Journal" note. The final Soulstone in Storm is after the Ice Canyon. There's a ladder after you defeat a group of soldiers, and waiting at the top is a powerful Pell Los Centurion enemy. Once you take him out, follow the left path for the final Storm Soulstone.
Embars Ruins
The first Soulstone in Embars Ruins is past the Corner Leading to the Abandoned Plains Nexus. You'll need to break a barricade past a horde of undead enemies and follow the path to the Soulstone. The second is past the Forgotten Temple Entrance Nexus where eventually you'll run into a Berserk Dragon's Talon enemy. This enemy can be a little tough if you're unfamiliar, but it is a great time to practice your Reflections or counter-attacks.
If you don't have an available spirit point to spend on a javelin throw, check your inventory for some Spirit Ore.
There are some vases behind the enemy you can destroy to reveal the Soulstone. The final Embars Ruins Soulstone is found at Depths of the Forgotten Temple during the main progression of the area. After turning the statues to solve the puzzle, you can throw a javelin at the Soulstone on the platform.
Phraugh Village
The first Phraugh Village Soulstone is at the Cabin on the Cliff area. Take out the Wild Boar and look for a cave entrance. Follow that to the end and climb the ladder, where you'll find the Soulstone at the top near a small tent. The second Soulstone can be found Phraugh Village Dock after taking the first lift. You'll open the large wooden gates, where there will be another large wooden structure, and behind some barrels is the Soulstone. The last one is at the Central Storehouse Entrance just outside the door.
Cradle of Jars
There are two Soulstones in the Cradle of Jars area where you first meet the Jarling collector. Both are found near the Jar Observatory Nexus, one in the garden near an invisible Undead Assassin, hidden behind some jars on the left-hand side. The second Soulstone is found after collecting the sixth Jarling if you look to the right for a jar hidden behind a large wooden pillar.
Bhangau Residence
The first Soulstone in Bhangou Residence is found from the Bhangou Lot Nexus, in a destroyed house guarded by a ranged enemy. After taking him out, there's a Undead Vagrant inside, but the Soulstone is on the right side of the house. The second is found under a wooden bridge near where you'll also find the Jarling for this area.
Skoffa Cave
The first Soulstone in Skoffa Cave where you'll eventually face Khazan's tough boss, Aratra. It is near the Collapsed Mine Nexus, behind a waterfall after you cross a shabby-looking bridge. For the second Soulstone, start at the Poisoned Depths Nexus and go down the slope. Look down the ledge into a poison pool, and you'll see the Soulstone you can hit with a Javelin. The third Soulstone in Skoffa Cave is at the Bottom of the Cave, where you'll see a poison pool with two spider enemies. After defeating them, the Soulstone is just past where they were waiting.
Spiders' Tomb
The final area in Charnak Mountain Range is the Spider's Tomb, where there are only two Soulstones that I found pretty easily. The first is past the Dead Spiders' Lair Nexus after fighting the Granite Jocko, near some wooden planks if you explore past where the enemy was. The second Soulstone in Spider's Tomb is at Jocko Hill, up a staircase and past a group of Wild Jockos.

Overall, these 20 Soulstones are the first and most straightforward of the four major regions in The First Berserker: Khazan. As you advance farther into the different regions, they will start to be hidden in more obscure areas and will take a bit more work than most of the ones you found in Charnak Mountain Range. As mentioned, it's a good idea to take Health Recovery increases whenever possible, unless you find that you aren't leveling fast enough and aren't having an issue with maintaining your health bar during tough boss fights.
